DVA HHS page

Here you can review the services that are available to eligible recipients or find information about making a claim.

HHS Assessment Form

If you need to apply for HHS, a reassessment, or request additional services, this is the D3919 form that you’ll need to fill out and submit to DVA. If you need a hand, please get in contact with us, we’d be glad to assist.

NDIS Eligibility Assessment

Sometimes you might be suffering from a condition where you are not eligible for assistance under DVA legislation. It’s worth looking into the NDIS to see if you’re eligible for support.
